Does permanent eyeliner hurt and how does the treatment work?
Author: Tsvety Bita, permanent makeup specialist

Sensitivity during permanent eyeliner is individual. Most people describe the sensation as pressure, vibration or irritation rather than constant strong pain. Calm preparation and clear communication are important for comfort.
What it feels like during the treatment
The eyelids are a sensitive area, and it is normal for the eyes to water, twitch slightly or want to blink. The specialist works in small sections and takes breaks when needed.
The sensation is affected by tiredness, stress, your individual sensitivity threshold and the condition of your skin. It is not helpful to compare yourself with others, because reactions differ.
- Pressure and a fine vibration.
- Temporary watering of the eyes.
- Slight stinging in more sensitive areas.
- Short breaks when needed.
Preparing for your appointment
Contact lenses must be removed for the treatment. Discuss eye conditions, allergies, dryness, recent surgery and any eye drops you use in advance. The treatment is not done if there is active inflammation or infection.
On the day of your appointment, do not wear heavy makeup around the eyes. If you wear false lashes or have had a lash treatment, check in advance whether and when they need to be removed.
- Bring glasses if you usually wear lenses.
- Do not come with an active stye, conjunctivitis or severe irritation.
- Tell us about previous reactions to cosmetics and anaesthetics.
Infralash or classic eyeliner
The infralash technique places colour between the lashes and gives subtle density without a clearly defined line. Classic eyeliner is more visible, and its shape is tailored to the eyelid, the lashes and the way the eye opens.
Very long, lifted wings do not suit every anatomy. The preliminary design should be assessed with the eyes both open and closed.
- Infralash – natural definition.
- Classic eyeliner – a clearer made-up effect.
- The thickness is determined by the eyelid, not just by a photo.
Healing and when to seek help
Slight swelling and sensitivity are possible in the early period. The colour looks darker and softens after fine flaking. Do not rub your eyes or remove the flakes.
Severe pain, a sudden change in vision, significantly increasing swelling, pus or a severe allergic reaction are not normal healing and require urgent medical attention.
- Follow the hygiene instructions.
- Avoid makeup and contact lenses for the specified period.
- Do not use drops or ointments that have not been prescribed.
Important: If you have an active eye problem or unusual symptoms after the treatment, see an ophthalmologist.
Frequently asked questions
Will I be able to keep my eyes closed?
Yes. The specialist stabilises the eyelid and guides you on when to relax your eyes and when to take a break.
Does infralash look lighter?
Yes – it fills in the line between the lashes and usually looks more discreet than classic eyeliner.
Can I have the treatment if my eyes are sensitive?
It depends on the cause of the sensitivity. A chronic condition, an active problem or recent surgery requires a medical assessment.
The most accurate recommendation is made after an individual consultation and an assessment of your skin, natural hairs, shape and desired result.
